Lethbridge Society for Meals on Wheels

To Qualify for Meals on Wheels you:

1. Are unable to prepare your own meals because of age, disability or illness.

2. Must want the service.

3. Agree to pay $_____ for the meal.

4. Agree to receive ______ meals per week (minimum 3 per week)

5. Agree to pay for a minimum of 5 meals.

6. Agree to return the Meals on Wheels REUSABLE Meal Tray and REUSABLE Dessert Cup. Please wash them and have them ready for pickup at the time of your NEXT meal delivery.

7. Agree to NOTIFY the Meals on Wheels coordinator 48 hours in advance to cancel or restart your meal service. If you fail to give this notice, you will be charged for the meal. Call us at 327-7990

8. Pay monthly. Your monthly invoice will be delivered the FIRST MEAL DAY of EACH MONTH. Collection will generally be the FIRST FRIDAY of the month. Please have a cheque or the correct amount of cash ready. A detailed statement will be issued upon request.
